PLUS Vision Corp.'s objective in developing our benefit plan was to provide
benefits that would meet each employees individual need and be competitive.
We hired an outside consultant firm to evaluate the benefits of several
different organizations. What we discovered is that "fixed" benefit
plans dont always meet everyones needs. Therefore, we developed
a Cafeteria style plan that enables employees to create their own benefit
plan. PLUS Vision Corp. provides each employee with $350.00 per month to
purchase benefits from a variety of options. Listed below is a brief summary
of each of PLUS' benefits.
- Medical - Regence BlueCross BlueShield of Oregon
HMO (Point of Service) POS Plus Plan - Under this plan, you select a
Primary Care Physician who coordinates your family's health care, including
referrals to network-participating specialists.
- Dental - This
plan uses Participating Dental Providers. Benefits under this plan
equals $1,000 per person per calendar year, $50 per person each
calendar year (maximum three deductible per family), Preventive
Services are 100% paid (no deductible), Basic Services are 80% paid
and Major Services are paid at 50%.
- Provides for only $10.00 co-pay at participating pharmacies.
- Provides for only $10.00 co-pay at participating providers.
- Vision Plan - This plan provides coverage for eye exams, lenses, and
frames every 24 months. In-network services require only a $10.00 co-payment
when performed by a participating ophthalmologist or optometrist. Additional
benefit includes partial payment for lenses, frames and contact lenses.
- Short Term Disability
Insurance - This plan provides you 40% of your compensation with
a maximum benefit of $500.00 per week, lasting a total of 13 weeks (until
long-term disability commences).
- Short Term Disability Buy-up - This optional
plan provides your short-term disability to increase to weekly benefit
to 60%.
- Long Term Disability Insurance - This plan is designed to replace
a portion of your income (up to 60%) in certain circumstances involving
an illness, injury, or disability.
- Flexible Spending Accounts - Pre-tax dollars may be used to pay for
non-covered health care expense as medical plan payroll deductions.
Additionally, you may defer pre-tax dollars to be used for dependent
care.
- Life and Accidental Death and Dismemberment Insurance - $50,000 Death
Benefit - Employees may choose additional coverage for themselves and
their family at group rates.
In addition to the benefits listed above, PLUS Vision Corp. also provides
the following:
- 401(k) - Under PLUS Vision Corp.'s current
401(k) Retirement Plan, employees may invest up to 20% of their annual
pre-tax salary through convenient payroll deductions. Additionally,
any Cafeteria Pan dollars, which were not allocated to other options,
are deposited into your 401K account to provide PLUS Vision Corp. employees
with optimum retirement.
- Personal Time Off - Accrual begins immediately
and accrual rate is based on years of service.
Years of Service Accrual Rate:
00 - 02 years 10 hours per month 3 weeks
03 - 10 years 13.33 hours / month 4 weeks
10+ - years 16.67 hours / month 5 weeks
- Paid Holidays - New Years Day, Thanksgiving
Day, Memorial Day, Day after Thanksgiving (Floating), Independence Day,
Christmas Day, Labor Day, Floating Day (Determined by the Company)
